The patient visited our office complaining of pain on the right lower side of the jaw. After thorough examination and history taking we found out that the patient has undergone some fillings, crown and bridges so the patient was advised to take full mouth X-ray (OPG) we noticed the patient has severe attrition with all his existing dentition and a prominent midline diastema with respect to upper teeth after evaluation of the X-ray we found that the patient had a history of root canal some 15 years back. It looked like a periapical granuloma/periapical cyst so after CBCT evaluation and taking consent of the patient we extracted his lower 1st premolar and 1st molar all the precautions was taken since extraction was done after taking blood reports and consent from his physician since the patient has a history of a stent placed in his heart then we made the patient wait for 2 months for prosthetic rehabilitation. Patient was given an option of RPD / FPD / IMPLANT patient was inclined more towards implants as he wanted a completely fixed option.
So we planned a minimal invasive comprehensive implant from Switzerland with a surgical guide. Advantage of this system is minimal loss of blood , no stitches and no aggressive bone grafting or flap reflection is required. After placing implants we did intra oral Welding so that we could deliver prosthesis immediately so that patient can eat his day to day food.
Since the patient had lost most of his tooth enamel and dentin in bruxism we had to restore his occlusion back to normal so we advised the patient to go for full mouth rehabilitation of his entire existing teeth.
Patient was very cooperative and everything went smoothly so after a period of 2 months we could close the existing diastema and bring back normal function now patient is very much confident and loves smiling and eating all his heart desires.
Take away – it is never too late to restore missing teeth even if you have systemic problems like heart issues and diabetes. With proper diagnosis we can achieve beautiful results which will last long with proper post operative maintenance and care.
